Add a new patient

Use this screen to create a patient record before starting an OSDx examination. Fields marked with an asterisk (*) are required.

How to add a patient

  1. Enter the patient's Name.
  2. Select the patient's Gender.
  3. Enter the Date of birth. The age field displays the patient's age in years.
  4. Enter a unique Patient ID, or use the button beside the field to generate one automatically.
  5. Add the optional contact, diagnosis and phone details as required.
  6. Confirm the Doctor name and the displayed Device ID.
  7. Click Start exam to create the examination and proceed to testing.

Patient details

Name Required
Enter the patient's full name.
Gender
Select the appropriate value from the drop-down list.
Date of birth Required
Choose the month, day and year of birth.
Age (years)
Displays the age corresponding to the entered date of birth.
Patient ID Required
The Patient ID must be unique for each patient on this OSDx device. You may enter your own unique patient identifier, or click the green + button beside the field to automatically generate a GUID (Globally Unique Identifier).
PIN / ZIP Code
Optional postal code for the patient's address.
Phone number
Optional patient contact number.
Contact
Optional additional contact or address information.
Diagnosis
Optional clinical diagnosis or relevant note for the examination.
Doctor name
Select the examining doctor from the drop-down list. Use the refresh button beside this field if the doctor list needs to be reloaded.
Device ID
Shows the identifier of the OSDx device being used for the examination.
Check the patient identity and date of birth before starting an examination. This helps prevent results from being saved against the wrong patient record.

Patient ID

The Patient ID must be unique for each patient on the device. If you do not have an existing clinic identifier to use, click the green + button beside the Patient ID field. OSDx will generate a GUID, providing a unique identifier for that patient record.

Starting the examination

After completing all required details, click Start exam. OSDx will then open the examination workspace for the selected patient.

If the examination does not start, check that all mandatory fields have been completed and that the OSDx device is available.
